Annual Application Details

Thank you for your interest in applying for a grant from The Stork Foundation for Infertility.

As a charitable organization, we are limited by the amount of funds that are donated and therefore cannot offer grants to all who apply in a given year. Grants of up to $15,000 per family are awarded on an annual basis and are paid directly to the provider on behalf of the patient.

Our annual grant cycle runs August 1 – September 30.  All applicants will be informed of their application status by mid-November. 

Grant Eligibility Requirements

  • 18 years of age or older.
  • Annual household income of $150,000 or below.
  • Medical documentation required: A diagnosis of infertility must be provided by a Board Certified Reproductive. Endocrinologist or documentation outlining your necessity for fertility treatment.
  • Applicant must have little or no infertility insurance coverage, or have exhausted infertility coverage benefits.
  • Legal residents and reside in the United States.
  • Recipients must begin treatment within six months of receiving a grant.

To be considered for a grant, you must complete the following by the application deadline:

  • Complete the application in its entirety
  • Write a brief (1,000 words or less) story that highlights your fertility journey
  • Have a doctor complete and submit the required medical form

In addition to meeting the above requirements, applicants must also demonstrate overall good physical and mental health, and show financial stability including basic health insurance for mother/prenatal care, adequate living arrangements and means for child support. 

**Please Note: A clear background check and proof of income through recent W2 verification will be required for all final grant recipients. Grants will not be awarded for those who do not meet eligibility criteria as outlined above.

Note: We prefer that your fertility clinic complete the Medical Evaluation Form online. If they are unable to access it, you may download the fillable PDF for them to complete.

In that case, you are responsible for collecting their responses, entering them into the online form yourself, and emailing the completed PDF to Grants@StorkFoundation.org.
