Grant Eligibility Requirements
Do both partners need to be residents of the U.S.?
Yes. Both partners must be legal residents of the United States.
Do I need to have an infertility diagnosis to be eligible?
Yes. A formal infertility diagnosis from your doctor is required. Exceptions include same-sex couples, single applicants, or those recently diagnosed with cancer.
Are grants awarded only to couples?
No. Grants are open to all individuals over the age of 18, regardless of gender, marital status, or sexual orientation.
What is the maximum household income allowed?
Applicants must have an annual household income of $150,000 or less.
Do I need to have an infertility diagnosis to be eligible?
Yes. Grants are open to applicants with no coverage or limited coverage.
- If you do have coverage, insurance must be billed first. The grant may be applied to remaining costs, up to the grant award total.
- If it is determined that you have majority coverage, you will no longer qualify and will forfeit the award.
Grant Details
How much funding can I receive?
Grants are awarded up to $15,000. Applicants are responsible for any costs above the award amount.
What treatments or services are covered?
Grant funds (paid directly to your clinic or provider) can be used for:
- IVF and IUI
- Cryopreservation
- Cycle medications
- Embryo testing
- Donor embryo, egg, or sperm
Can grants be used for past procedures?
No. Grants cannot reimburse cycles or treatments already completed. Funds are available for treatment beginning on or after the grant award date.
Is there a timeline for using grant funds?
Yes. Treatment must begin within 6 months of receiving the grant. All funds must be used within 1 year of your treatment start date. Unused funds after this period are forfeited.
Application Process
Can I submit a paper application?
All applications must be completed online. If your fertility clinic has trouble accessing the Medical Evaluation Form, a downloadable fillable PDF is available.
- Important: Even if your clinic provides a PDF, you (the applicant) are responsible for entering their responses into the online Medical Evaluation Form so we have an official electronic record.
Is there an application fee?
No, our goal is to eliminate as many financial barriers for our potential grant recipients and the application process is free.
How many grants are awarded per cycle?
The number of grants awarded each cycle depends on fundraising and available funds, so it can vary from year to year. Since 2020, The Stork Foundation has awarded over $390,000 in grant funds to 38 families in need, helping make fertility treatment more accessible and possible. Please note that not all applicants will receive a grant each cycle.
Can I apply more than once?
Yes, applicants may apply more than once. You will need to submit an updated medical form and story along with your application for each new grant application cycle. Individuals and/or couples can receive a lifetime grant maximum of $15,000.
When will I hear back about my application?
Applications are reviewed in October, and you will receive an update via email by mid-November.
Does the application need to be completed by the deadline?
Yes. All forms, including the medical form, must be fully submitted by the deadline. If you encounter issues, email Grants@StorkFoundation.org.
